Qualifications:

-US citizen

-Minimum 3.5 GPA

-Demonstrate significant accomplishments and showing that at least one award was earned in one of the following categories: athletics, performing arts, visual arts, science, or community service.

-Must complete application and include transcripts, recommendation letter, story of excellence, and copy of award (all is explained in the application)

-High school student with asthma who has been accepted to an accredited college or university in the United States

Amount: Varies

Number of Recipients: Two recipients for each of the five categories

Degree Type: Undergraduate
